Hours:	M - W: 8 am-4 pm; Th: 8am-7pm; Fri: 8 am-3pm. We take your calls 6 days a week! You can reach us M-Th: 8am -7pm, F: 8am-5pm, S:7am - 3
Eligibility:	Services available to anyone who requests them regardless of age, income, marital status, or residency location.
Fees:	Sliding scale; cash; major credit/debit cards; money orders are accepted. If you are uninsured, you may qualify for state-funded program, bring birth certificate, pay stub, photo ID, proof of residence SS Number,to first visit.
Intake Process:	Walk-in, telephone and by medical appointment. Appointments can also be made online at www.ppil.org
