Eligibility: Persons currently under a doctor's care due to an acute or life threatening physical or mental disability; has a pending case with the Social Security Administration for SSI or SSD.

Intake Process: Come in & pick up application; once application is completed, an appointment must be processed for eligibility purposes. If the applicant meets qualifications for eligibility then an appointment will be given to meet with a caseworker.

The City of Champaign Township does not provide RENTAL, UTILITY or any other CASH ASSISTANCE. The City of Champaign Township Supervisor is state mandated to provide General Assistance-Transitional Assistance. Applicants must obtain a doctor's statement indicating that the applicant's condition is life threatening and they are unable to work. Applicant must not be receiving any other income from any other agencies or sources. The grant amount is $245 per month. Adult cases only.
