Program/Service Notes:

Public Health District office registers all births and deaths in the county. After 10 years, birth and death records may be obtained from the Champaign County Clerk. Birth certificates are $11.00 for the first copy and $3.00 for each additional copy per person purchased at he same time. Death certificates are $15.00 for the first copy and $7.00 for each additional copy per person purchased at the same time. For information on obtaining a birth certificate for a person born in other counties in Illinois, contact Division of Vital Records, IL Dept. of Public Health, 605 W.
